Our Services

Residential Services

The Fort St. John Association for Community Living provides a variety of services varying from total care to individuals with high medical needs to semi-independent living for those who just require some services. The number of individuals living in the homes vary, as well as the level of support required. Ongoing training is provided to enable staff to provide quality support to all individuals in care.

Respite

For families of adult children with special needs, respite means they get a break knowing that their loved one is in a safe, healthy and fun environment.


Day Programs

Community Connections

This service provides community access, supported independent living and life skills training to individuals with developmental disabilities. This includes skills training such as budgeting, cooking, laundry and housekeeping.

Supported Employment

Careers is a program helping individuals obtain and maintain work within the community. On the job training and support is provided to both the individual and employer. Resume writing, application writing and other employment skills are offered.




How do I apply for Services?

  • You need to meet with your Community Living BC (CLBC) representative to determine whether you qualify for services. Once they determine that you are eligible for their sevices you need to plan for what services you may require.
  • Come and see our facilities to see what services may fit your needs. We will give you an intake form (one is provided in this package).
  • Fill out an intake form that is provided by our agency. This includes a medical form that is to be filled out by your family physician and a current risk assessment that you can fill out.
  • The CLBC worker, once they have the intake form then contacts the Executive Director who will review the intake form with the appropriate supervisor.
  • After the Executive Director and the appropriate supervisor feel that they may proceed with the referral after reviewing the above noted criteria; a meeting with the referring CLBC worker, individual and the family (or support network) will be arranged.
  • At this meeting the individual and family will become familiar with the program, the facilities, and the staff. Any outstanding questions will be addressed. A firm decision on whether or not to continue with the intake will be made.
  • At this point, if the referral is accepted, a Person Centered Plan (PCP) should be started for the individual to set goals and outcomes for our services. Also, an individual care plan will also be started based on individual needs eg. personal care (if any required), diet, etc.

**Admissions will be dependent on vacancy. Priority regarding placement will be given dependent on needs, in consultation with Community Living BC.
